The RN Case Manager is responsible for independent management of the Hospice patient population requiring advanced assessment, teaching, and decision-making skills.
The RN Case Manager cares for a caseload of hospice patients by evaluating the patient for appropriateness of hospice care and developing the plan of care in collaboration with the provider, caregivers, and Hospice Interdisciplinary Team members.
The RN Case Manager follows up by evaluating the effectiveness of the plan of care and monitoring the patient and caregivers' response to the plan.
The RN Case Manager provides patient care based on the provider's orders, departmental policies and procedures, regulatory guidelines, and best practices.
The RN Case Manager collaborates with the provider, patient caregivers, and Hospice Interdisciplinary team members to meet the goals of care while considering the patient's limitations and personal preferences in accordance with hospice philosophy.

Required Licensure / Registration / Certification:Current Texas State license as a Registered Nurse, CPR certified, and current Texas Drivers License and auto insurance.
Must have reliable transportation.
Required Experience:Two years experience as a Registered Nurse with med/surg experience.
